For this year's Gift and Volunteer Opportunities Survey, you may download an Acrobat PDF form (click link below) or by paper form. Your willingness to use the PDF form will contribute to time saving in the office.

Note: After you download the pdf form, you can fill it in and save it on your computer - use the Save (diskette symbol) on the Adobe Reader tool bar. Then simply send an email to office@firstcrc.org and attach that pdf file. You can also print that pdf file for your records.

Details

  • Please fill out the form to indicate your gifts and interests in volunteering for the next year.
  • We encourage all members from middle school through adults to fill out the annual form.
  • Check all items in which you are interested for the 2009-2010 ministry year.
  • You will be contacted regarding your availability when there are needs in the areas you have checked.
  • Please submit by June 5, 2009.
